Chose your farm by browsing MACSAC’s Farmlist. Be sure to read through the Factors to Consider When Choosing A CSA to help you find the right farm for your family.
Contact the farm directly to request a brochure, confirm pick up locations and times, and ensure there are still shares available for you to purchase.

PHYSICIANS PLUS EAT HEALTHY REBATE

The Eat Healthy Rebate program is part of Physicians Plus Good Health Bonus program. The total maximum annual reimbursement under the Good Health Bonus program is $100 per year for single contracts and $200 per year for family contracts.

Requirements for Eat Healthy Rebates on your CSA Share:

• You must be a Physicians Plus member at the time your produce share is purchased.
• Only Fresh Produce (vegetable and fruit) shares area eligible for reimbursement.
• You must make a minimum personal payment equal to or greater than the Rebate amount for which you are eligible ($100 for single contracts, $200 for family contracts).

Follow these steps to claim your rebate:

1. On your MACSAC Farm sign up form please write:

• P+ Eat Healthy Rebate
• Your date of birth
• Your phone number

2. Send your farm sign up form and payment to the farm according to the instructions on the form.
3. Send a copy of your form and proof of payment to:

As part of the Wellness Incentives Now (WIN) program, Dean Health plan will reimburse individual members up to $100 and family members up to $200 each contract year for healthy lifestyle endeavors including a produce share from a MACSAC farm.

Requirements for Eat Healthy Rebates on your CSA Share:

• You must be a Dean Health Plan member at the time your produce share is purchased.
• Only Fresh Produce (vegetable and fruit) shares area eligible for reimbursement.
• You must make a minimum personal payment equal to or greater than the Rebate amount for which you are eligible ($100 for single contracts, $200 for family contracts).

To receive a reimbursement:

• Send your farm sign-up form and payment to your farm according to their instructions.
• Print out and complete your Dean Health WIN Reimbursement Form.
• Send a copy of your MACSAC farm sign-up form, your WIN reimbursement form, and  proof of payment to the address below:
