MACSAC and our Health Provider Partners want to make CSA even more rewarding and accessible...

The health provider partnership is much more than just eating healthier foods. It is a joint effort between area HMO's and MACSAC to encourage sustainable farming, keep local dollars within the community and to build a healthier Wisconsin.

The following health insurance companies are offering a cash rebate off the cost of produce shares from MACSAC farms for the 2008 season. Click on the links below and learn how to get paid to join a CSA.

PHYSICIAN'S PLUS EAT HEALTHY REBATE

How it works:

  • The Eat Healthy Rebate program is part of the Good Health Bonus program. You can choose to apply your Good Health Bonus in the way that best fits your lifestyle: to approved classes, to Weight Watchers, GO-TO Healthy Choices or to the Eat Healthy Rebate. The total maximum annual reimbursement under the Good Health Bonus program is $100 per year for single contracts and $200 per year for family contracts.
  • You must be a Physicians Plus member at the time your produce share is purchased.
  • To be eligible for the Eat Healthy Rebate, you must order a produce share (meat, eggs and dairy purchases are not eligible for reimbursement) from a MACSAC farm and make a minimum payment (excluding public or private assistance programs) equal to or greater than the Rebate amount for which you are eligible ($100 for single contracts, $200 for family contracts).

Getting started:

  • Learn how Community Supported Agriculture works by reading "About CSA"
  • CSA members sign up directly with farms. Start choosing a farm by browsing our Farmlist with farm descriptions and contact information or read through "Factors to Consider When Choosing A CSA. If you are planning to join s farm in the fall you'll want to view the list of farms offering late season and winter storage shares.
  • Contact the farm diectly to request a brochure, confirm dropsite locations and times, and confirm that they are still have shares available for the season
  • Write "P+ Eat Healthy Rebate," along with your date of birth, on your MACSAC farm sign up form. Please be sure your phone number is on the form too.
  • Send your farm sign up form and payment to the farm according to the instructions on the form.

As part of our "Live Well-Wellness Reimbursement Program" GHC-SCW will reimburse members up to $100 and families up to $200 each contract year. Follow these steps to receive your rebate:
  • Learn how Community Supported Agriculture works by reading "About CSA"
  • CSA members sign up directly with farms. Start choosing a farm by browsing our Farmlist with farm descriptions and contact information or read through "Factors to Consider When Choosing A CSA.     • 2 GHC-SCW member number
    • 3 Their date of birth
    • 4 Phone number
  • Member sends copy of MACSAC farm sign-up form to Group Health Cooperative.

DEAN HEALTH PLAN

As part of the Wellness Incentives Now (WIN) program, Dean Health plan will reimburse members up to $100 and families up to $300 each contract year for a variety of healthy endeavors, including a produce share from a MACSAC farm.

UNITY HEALTH INSURANCE

Unity reimburses subscribers with single coverage up to $50 per calendar year and subscribers with family coverage up to $100 for purchasing a produce share from a Madison Area Community Supported Agriculture Coalition Cooperative farm between January 1, 2008 and December 31, 2008.

To be eligible for reimbursement, you must:

• Be a Unity subscriber at least 18 years of age
• Have either single or family coverage with Unity
• Purchase a produce share from a Madison Area Community Supported Agriculture Coalition Cooperative farm
• Complete and mail the attached coupon with your receiptPlease note: The receipt must include the farm’s name, address,
telephone number, a description of the purchase, date of purchase,
and the amount paid. Members may use a copy of their CSA farm sign up form to serve as a receipt.To receive your reimbursement, complete the following steps:
1. Fill out the Unity Health coupon
2. Attach your receipt
